Streamlining Our Process: New Consolidated Fabrication & Chart Field ST Request Form

We’re excited to share an important update designed to simplify workflows, improve visibility and make life a little easier for everyone involved in submitting and tracking requests. To enhance efficiency and create better alignment across teams, we have consolidated the Fabrication Request Form and the associated Chart Field ST Request Form into one unified DocuSign form.

Important Clarification

The Chart Field Request Form referenced in this update applies only to ST requests related specifically to fabrications. This change does not impact other types of Chart Field requests outside of fabrication-related ST activity.

Why the Change?

Over time, we recognized that having two separate forms for closely related fabrication and ST requests created unnecessary duplication, extra follow-ups, and occasional tracking challenges. By combining them into a single DocuSign workflow, we are creating:

  • Improved efficiency – Submit everything in one place.
  • Better tracking and transparency – Streamlined routing and clearer status visibility.
  • Reduced processing time – Fewer handoffs and fewer delays
  • Stronger data accuracy – Minimized risk of mismatched or incomplete information.
  • Greater synergy across teams – A more cohesive, end-to-end request experience

What This Means for You

Going forward, all fabrication-related requests that require an ST Chart Field should be submitted using the new consolidated DocuSign form.

The access site remains the same, so there is no change to where you go to initiate your request, only the form itself has been enhanced and streamlined.Ìý

The updated form:

  • Guides you step-by-step through required fields.
  • Ensures all necessary financial and fabrication details are captured at once.
  • Automatically routes to the appropriate approvers.
  • Provides a cleaner audit trail for reporting and compliance.

When Does This Take Effect?

The new form is available now and should be used for all new fabrication submissions effective immediately. Please discontinue use of the previous standalone fabrication and related ST forms for these requests.
